The problem (but not the solution) seems to be clearly indicated in the 
tapeinfo output (very good of you to include it !!!).  It is the 
tapealert[32], which indicates that you have some kind of hardware problem.  
You can look up tapealert 32 on the web and probably find a lot more 
information, or ask your tape vendor.  My best guesses are:

1. SCSI not properly terminated.
2. Cables not well screwed down.
3. Cable too long.
...

Dell support should be able to give you plenty of help -- just keep insisting 
that it appears to be a hardware problem (not a Bacula software problem as 
they might claim) because of the I/O error Bacula got (originates in the OS 
software), and the tapealert (comes from the drive itself).
