Your problem description looks like the autochanger setup might be faulty. Have you tried the btape tests for the autochanger, and can you use mtx-changer from the console? Also, make sure the catalog reflects the actual autochanger inventory. Check using the query to list the volumes in the autochanger, and, if that's not correct, use 'update slots [scan] storage=overland'.
