It looks like a hardware problem.

Does "stopped everything" mean shutdown and power off/on?

Can you run the HP "Library and Tape Tools" diagnostics
(http://h18000.www1.hp.com/products/storageworks/ltt/index.html)?  That should
help to identify hardware problems.
