Hello, Some place in the manual, there is a checklist of what to do before putting something into production.  Though it doesn't explicitly say this, it should also be used if you change some significant component such as the Autochanger.  One of the tests is to run btape and specifically the "test" and "autochanger" commands.  Doing that would very likely have identified the problem. Unfortunately this advice is too late to help you, but hopefully it can help someone in the future.
