Why do these papers refer to only three categories?

John Hick lists five (six if you count atheism as a nota response).

The different responses to religious diversity:
Exclusivism
Inclusivism
Pluralism
Relativism
Subjectivism

Atheism, Agnosticism, Humanism, Secularism, Nonreligion, Irreligion, etc. as a 
None of the Above

The papers so far have been ignoring Relativism and Subjectivism.
