My newborn has an undescended testicle. What is it,
and how common is this condition?
It's not unusual for a baby boy,
especially one who arrives early, to enter the world
with one or both testicles
undescended into the scrotum. That's why your
healthcare provider will thoroughly
examine your baby's genitals during the initial
exam to confirm that both have
dropped into the scrotum or at least into the canal
just above it. If they haven't, the
condition will usually correct itself within the first
year.
What causes it?
No one knows for sure, but it's
believed to occur around the seventh month in
utero since that's when a fetus'
testicles normally begin their descent from the
abdomen into the scrotal
sac. The American Academy of
Pediatrics' book, Caring for Your Baby and Young Child
cites the following
possible reasons for this
condition:
� There may be insufficient hormones from the mother
or from the developing testicles to stimulate normal
development
� The testes themselves may be abnormal in response to
the hormones
� A physical blockage may have prevented normal
descent
� Hormones taken during pregnancy may affect a male
fetus' testes
Can it be corrected?
Luckily, the condition corrects
itself in about two-thirds of cases. If both testicles
haven't descended by age 1 or 2, the child will
require treatment. Two treatments are used for
undescended testicles:
� Hormone therapy stimulates the testes to increase
the testosterone level. This is usually done by
administering twice-weekly
injections of human chorionic gonadotropin, a hormone
that stimulates the
maturation of the testicles and
jump-starts their downward migration over the course
of several weeks. This
treatment is effective in about 20
percent of all cases. The lower the undescended testis
is, the better the
chances of the treatment is of
succeeding. In the event that it's unsuccessful,
doctors will resort to surgery.
� Surgery is used if a basic abnormality is the cause
of the problem. For example, if a band of fibrous
tissue blocks the testicles'
descent into the scrotum, the tissue must be removed
surgically. If the ligament
that supports the testicles is too
short, it can be lengthened. Surgery is also used if
the condition is
accompanied by an inguinal hernia,
a common complication in which a loop of the child's
intestine pushes
its way into the canal or even down
into the scrotum. These are minor outpatient
procedures.
What happens if it's left untreated?
Testicles don't seem to mature
normally when they aren't in the scrotum, so the risk
of testicular cancer
and infertility is increased if
undescended testicles are left in place for more than
two years, says the
AAP. In very rare cases, an
undescended testicle can become twisted, cutting off
the blood supply and
causing pain in the groin or
scrotal area.
In any case, as long as your son's
testicles are corrected within two years (either on
their own or with some
intervention), there's nothing to
worry about.
