Did you check the concurrency number on all daemons. 
>From what I've checked here it's 1 = 1 compared to what bacula offer.
if Maximum Concurrent Jobs = 20 is everywhere you got 20 connections pipe.

Just about point 3 : are you sure you didn't get full partitions ? 
and sometimes a volume expire, allowing the backup to proceed a bit more ?
