I think most all of that could be done by creating a stationery file. I use them to create skeleton PHP and Unix Shell files, among others. Just create a template file like you want and save it to ~/Library/Application\ Support/BBEdit/Stationery. Make sure you check "Save as stationery" when you save it. To use it select it from the "File -> New with Stationery" menu.
