My brute force solution: Trash the folder at ~/Library/Preferences/com.apple.LaunchServices and the file at ~Library/Preferences/com.apple.LaunchServices.plist. Then rebuild the LaunchServices database (I used the free Onyx.app utility for that) and reboot. WARNING: Doing this will reset all your LaunchService prefs to OS X defaults. But it was driving me nuts enough to take drastic measures. Hope that helps. (Edited Feb. 15 to correct filenames.)
