Can you tell us exactly what the failure mode of bcm43xx-fwcutter is?
e.g. What is written to stdout when you try to extract the firmware?
There are two major versions of the firmware, version 3 and version 4,
and the driver in the mainline kernel can only understand version 3.
The fwcutter tool, however, should understand both but warn you about
trying to use version 4 firmware.

Also, the initialization messages from the driver would be helpful.
Load the driver and then send us the last several messages printed in
'dmesg'. Specifically, this should tell us exactly what model of
Broadcom chip you have.
