1. I don't know if MACTIME gets set or not. How do I check that?
2. ACKs and beacons are received perfectly, yes. Not sure about
RTS/CTS, although I do remember seeing RTS packets in a pcap file I
made with the card.
3. The locally-sent packets arrive back correctly, both those that
have been generated by the system on wlan0, and those that are
injected through Radiotap.

Also, disabling HW crypto fixes the problem.

My proposal: We should automatically disable HW crypto when the card
is operating in multiple-interfaces mode, or if there is a monitor
interface present.
