If you try load a web form directly as a modal form box it will open up in a new 
window after being posted back. To overcome this you need to display the web form as a 
part of FRAMESET. 

Create a dummy html page with the content as follows

<html>
        <head>
                <title>Header Frameset</title>
        </head>
        <frameset rows="0,*">
                <frame name="header" src="" 
                scrolling="no" noresize>
                <frame name="main" src="MyDialog.aspx">
        </frameset>
</html>

and try to open the html page as a modal form
