Despite the advice to store the form data in a database instead of a flat file, I finally set up the script the way I had in mind, with a directory structure created as needed on my web server and the information in a flat file. It works great.
Of course, now that my husband can actually see this in action he has a grand vision for turing this into a full-fledged application instead of just an online version of one form, so I really will need to install and use mysql. Some day... this is all in my "free time" anyway. So I wanted to say thanks to the group for their help, especially the regulars. If there is anyone else needing help on this same topic (original posts would be the archives from the end of September) please let me know. And if anyone else is doing intranet applications for a recording studio - and trying to make them musician-proof :) - we may need to form a support group.
