You need to define a much smaller test/problem. Here's some questions to give you a start: Have you gotten ANY cgi to work? Have you gotten ANY perl cgi to work? Forgetting CGI for a moment: Have you proven your "database" works? Have you written a perl script that correctly appends to the "database"?
