I didn't use this with perl, it was with php.
however, I used

<INPUT TYPE="radio" NAME=answer[1] VALUE="blah">Blah
<INPUT TYPE="radio" NAME=answer[2] VALUE="blah">Blah

Then in the php I had a loop that went through the values of answer
