No luck!  I get the following:

Can't locate Date/Calc.pm in @INC (@INC contains:
/export/home/kthmgr/lib/perl5/site_perl/5.6.0/sun4_solaris
/usr/local/lib/perl5/5.6.0/sun4-solaris
/usr/local/lib/perl5/5.6.0 /usr/local/lib/perl5/site_perl/5.6.0/sun4-solaris
/usr/local/lib/perl5/site_perl/5.6.0 /usr/local/lib/perl5/site_perl .) at
fo_forms.pl line 1001.

Any other ideas?
