If you are still looking to Perl for this functionality, you should have a look at the GD module and the PerlMagick module.

Like the Gimp you might be able to use one or both of the modules along with Gtk Perl to handle the images on the fly. Though it might be easier to use the Gimp's perl plugin capability though I have not used it myself.

GIMP (v1.2.3 at least) includes such a feature, if I'm understanding you correctly. Filters->Render->Pattern->Grid
