What about checking for a 'cgi_error'. It appears that if something happens during 
upload this will be set and may provide more information.  Again, what does your form 
request look like? Can you print the key/value pairs of the HASH reference returned in 
the $info so we can see what it has in it.  How about turning on use strict just for 
the kicks, does it complain about anything?
