I tried inserting some lines into the form from the cities demo form, but
that didn't help.
Can someone tell me what I'm missing?

Although the Qt layouts make it more straightforward to generate forms by
hand, I've found it useful to add a test function to the script to make sure
I'm on the right track. That way I can separate the initialization logic
that is usually in the run function. For example:

coclass'example'

EXAMPLE=: 0 : 0
rem form definition
)

test=: 3 : 0
wd EXAMPLE
wd'pshow'
)

Running test_example_ in the terminal brings up the form and if need be I
can go back and adjust the form definition. The usual run or create function
contains all of the necessary logic to actually use the form.

Although the above is probably obvious, once I started using this approach,
building forms was much quicker and I stopped missing the form editor.
