Did the scan complete? That looks like a fatal error. The trouble with the logging is that DBIx creates a prepared SQL statement (all the ?, 's) but when the INSERT fails it neglects to log the data that is being passed to the statement.
It appears to be choking on an audio file, but one that is pointed to by a CUE sheet. Do you require CUE sheet parsing?
