The album art problem appears to be a scanning issue. SqueezeCenter has twice abruptly stopped in the middle of the "Merge Various Artists" phase (e.g. at 59 of 230), but then incorrectly reports that it has finished scanning. This happened when I initially installed SqueezeCenter; in that case, I was using a UNC path (\\office\music_1) for the music folder. I then mapped the network drive to z: and rescanned, and it finished the "Merge" phase, then followed with "Artwork Scan" and "Database Optimize". When it finished, the artwork appeared properly.
I tried going back to the UNC paths, but the scanner never got through the directory scan, and I had to kill it after 20 minutes. I rescanned using the mapped drive, and I got the abbreviated scan, and no cover art. Finally, I rescanned one last time, successfully.
