This has been an on and off problem for Last.fm users using third party scrobbling applications for a while now. Last.fm say there's no pattern to what songs are duplicated and why, and they believe the problem is being caused by scobbling plugins that are out of date. Doesn't explain why the plugins work sometimes and not other IMHO, but there you go.
I think (touch wood) I have managed to fix this on my system by deinstalling the Last.fm Squeezescrobbler plugin, and doing a fresh install of the lastest version from James' site (I was running a version from about 4 or 5 months ago, including inputting all my passwords again. Haven't had a multiple scrobble since (over 2 weeks ago).
