Run a hard drive fitness test and make sure you're not running into silent data corruption! Hopefully it's nothing big, but that looks like the typical symptoms of potential data corruption.
Download and burn this ISO: http://www.hitachigst.com/hdd/support/downloads/dft32_v414_b00.iso And then boot your computer off of it. Do the advanced test.
