The web interface was showing no progress and the log stops at 1557. 
Also, being missing most of the artwork led me to believe the precaching
failed.  Maybe it's just the progress updates that fail, which may
explain some of the other problems I see when watching progress in the
web interface.

I ran a second clear/rescan immediately after and it completed in a
couple of seconds and did nothing.

I then ran a third clear/rescan and it ran successfully.  In that scan,
the log shows progress beyond 1557:

That wasn't really the question.  I haven't yet tried to run a scan
without precaching.  The question was even without precaching, or if
precaching fails, shouldn't you still get artwork?
