I remotely logged into the Win2k3 server and it informed me that resources were so low that it couldn't even load my user profile. I've never seen that before on this server, and only ever on a server when something has seriously gone into the weeds. I rebooted the server and was then able to run a clear & rescan.
Similar problems are being reported in another thread with the 7.5.4 release candidate.
