Are you running a 'clear and rescan everything', or is this happening on
the initial startup of the server with an empty database?

The places to look for an explanation are the server.log and
scanner.log files. If the server is still running you can access the
logs in a web browser through Settings > Information, and the links at
the bottom of the page.

If you can't get past this problem you may have to shut down the server
and then delete everything in the cache folder (it's location is shown
on the same Settings > Information page). When you restart the server
it should see that there's no database and automatically launch
server.exe to begin a scan of the library.
