You could try removing the plugin preference file (extendedbrowsemodes.prefs).Yes, this helps.
I doubt it. Most likely it's just a bug which needed a server restart to get cleared out. The prefs file should have nothing to do with the Contributors menu item. I'll double-check. Thanks for bringing this up!
