If LMS is running on Windows and you've recently gone through a Daylight Savings Time change, you're probably seeing the effects of a Windows oddity following such a time change. The last modified time of every file will appear to have changed by one hour, so LMS will rescan every file in the library the first time you do a new & changed scan. Because of how slow that is, it's best to run a full clear & rescan after a time change.
