It can use any image, but it only saves the one that is directly referenced and forgets the other images if they aren't used. Right now it is pretty easy for me to switch MatCaps the way it is but I believe they may be an objection to saving images that are used purely for MatCaps (or Image Icons) as part of a .blend.
Another thing is, MatCaps are special images with a particular format and having all images present from the .blend is actually a bit of a distraction IMO. For those reasons I think it would be better if the selector only referenced external files but I have not implemented it that way atm. Right now it treats all images loaded as matcaps as temporary except the one in use when the file is saved. The is actually the same behavior you would get if you created a matcap material and changed the image over and over while exploring mat caps. So actually, the way it behaves right now is almost exactly the same way as if you did the 5 or 6 steps to set up a matcap material and then started switching images. It currently does allow you to select any image loaded into blender. What you seem to be objecting to is making it even easier to reference a matcap by loading an *entire directory* of matcaps at once into the selector without having to open the file select dialog. Actually I guess what it really is is that regular images might not be referenced this way.
