I have subfolders in my papers folder. It is a relic from pre-bibdesk
days. One thing it is good for is that sometimes I will clean things
out, deleting bib records from old projects or those that have been
replaced with better ones that haven't been found in duplicate
searching. It is easy to spot something that's out of place---a
folder for an author I thought I didn't have, or something outside of
the hierarchy altogether.
As for naming the files, I use the following:
%a1/%f{Cite Key}.pdf
This give a folder in the papers folder with the author's last name,
then a filename that has the cite key.
This is good because when you've used the reference in LaTeX source,
you can easily find what the reference refers to, since the cite keys
are unique.
