BibDesk has settings for all of these when you configure a z39.50  
search group (I suspect LoC gateway is the hostname of the server).

BibDesk does not support username/password for proxies.  Are you  
dealing with an EZProxy, or is this a general proxy for network  
access?  If the latter, can you set it up in the system's Network  
preferences?  Without further info it's really impossible to do  
anything.
