(3) Being my personal hobbyhorse: I think that right-turn-on-red was the seed for current ped hostility in the US. Boston doesn't have RTOR (they did have to put up a whole bunch of signs, naturally).

I agree.

The city of Platteville has placed a sign at every downtown crosswalk which reads, "STOP for Pedestrians." (State law in WI only requires the unenforcible "Yield".) Madison should do the same at every busy crosswalk in town.
