Unless it's been fixed in the last year, I can tell you one problem location:
Commonwealth Ave at Monroe.

As far as I can tell, there is a loop detector far up Commonwealth, but it doesn't pick up bikes. I have sat at this intersection several times waiting for a car to trigger the signal. The really annoying part is that if the car doesn't proceed to Monroe - if the driver turns off into a driveway or another street, the cycle is interrupted, and I will never get a green.

Here's how I know:

I'm waiting for a green to turn onto Monroe. The ped light to cross Monroe is a solid WALK. A car starts down Commonwealth. The ped light begins to flash DON'T WALK. (Ah, I'm going to get a green, I think.)
Car turns off somewhere a block or so away.
Ped light goes back to solid WALK.
Repeat with three cars in a row.

Other than that, things work very well!
