Hi! I'm supposed to develop a good mobility questionaire for my community (all modalities) to start to try to develop a plan for the city. I'm looking for sites that might have good models of questionaires available for copy/translation. I need to start fairly simple but want to make sure that it isn't *just* about bikes and doesn't forget special needs (I oughta take some shots of the sidewalks around here - you'd never get a wheelchair down some and, frankly, I've NEVER see sight impaired folks trying it alone here).
I hope to offer it via street interviews, mailings and via the Web. Any good places to grab such models?
