Quite honestly, I find that anything that requires me to remove my hands from the handlebars and find a switch/lever/button to release a loud noise in an emergency - like being cut off in traffic or telling a clueless driver that s/he is about to hit you - is too much time and trouble. In those situations, my voice is much more readily accessible and instantaneously activated.
In some large cities (I have observed this in Chicago) bicyclists ride with a whistle clenched in their teeth. I'm not sure how they manage this without either taking all the joy out of biking or having some adverse physical reaction, such as drooling or gasping for air. But then again, I've never tried it.
