Andy,
I have tried both subscribing as well as the LIST command - all BINC
seems to see is INBOX when there are three other folders that exist,
are in Maildir format and can be seen by Courier.
Other ideas?

You may want to check the basic functionality test on lifewithbincimap.org
- try the LIST command and see if the other mailboxes are seen by Binc
IMAP at all :-).
Otherwise, I'd try subscribing to them, like the others have hinted.
