I just took this survey, and it's actually pretty inclusive of sexuality, including allowing answers for those who have *no* sexual attraction but still label themselves queer. It's somewhat trans inclusive in that they have "male, female, trans" as their answers to "What is your gender?" but a lot of the questions involved ask about sexual arousal, attraction, and experience with males or females.
