adding the All Workspaces menu entry

I started looking into implementing an "All Workspaces" entry on the Workspaces
menu.  The problem is that there is no clean way to do this.  Each workspace
provides its own window list and menu.  So I can either keep a menu full of
duplicate entries or I can dynamically generate the menu each time the user
asks for the workspaces menu.  Neither sounds apetizing.
