I just figured it out - it was some weird problem with my viminfo
file. I was using a little trick I found that has vim automatically
detect the last line I was editing when I last opened a file.

I'm not sure what the specific problem was, but it seems like Vim was
writing out information about an empty buffer so when QS was launching
vim, it was opening a blank buffer on top of my file. As you can
tell, I'm not positive what the issue really was, but it's gone now :)
