QS by default only searches ~/Documents down 2 levels (maybe 1, I forget). This is to keep the catalog small and QS fast. I don't know what you mean by "I selected everything to be cataloged" so it's hard to know what it's set to do.

You might find the Catalog section of the QS manual helpful. It has screenshots and describes why you don't want to just set QS to catalog everything to infinite depth.
