I can confirm this bug. It seems that QS is getting confused when
Escape is pressed while the focus is in the third pane, and when
reinvoked it brings up actions that would be appropriate if the former
indirect (=third pane) object were the direct (=first pane) object of
the command, even though the same direct object is still in the first
pane.

First, Danijel, please register this as a bug at
http://code.google.com/p/blacktree-quicksilver/issues/list

Second, until a fix is issued in a future build, a workaround is to
press ← to move up one level in the file hierarchy and then → to re-
select the item. By re-selecting the item, you will reset QS's choice
of appropriate actions for the item. Instead of the arrow keys, you
can also use any other method of re-selecting the item, such as typing
in the first pane to bring it up again, by using one of the various
methods of getting the Finder selection, etc.
