Quit Quicksilver, navigate to ~/Library/Caches/ and delete the Quicksilver com.blacktree.Quicksilver folders, then launch Quicksilver. It will rescan your catalog and everything will be OK. On a somewhat related note: which version are you using? I have seen this problem with older builds, but it has not come up with the newest.
