I mostly use the menu items for things like getting to the 'open recent'
menu items which changes regularly (not HenningJ's 'recent items' fix
doesn't work for XCode.
I agree that it can take a while (I'm on an older machine and for XCode it
takes 4s), so we should look at trying to streamline the process.

Neurolepsy is the main guy working on the UI Access plugin. I'm not too sure
what goes on inside it, but I think he did say it uses older Carbon stuff,
and he might look at changing it to Cocoa which would likely speed things
up.
