Deleting all references to "tv" in the Mnemonics.plist file should remove ttv.com as the default as you've correctly guessed. Have you tried 'increasing' or 'decreasing' the score for the two items (click the dot next to the entry in the results view)
Are either of the entries that are causing you problems connected to the 1Password module? It had a previous bug that meant the 'set as default' but would not always work.
