There are several bugs in the iCal plugin at the moment. We should be releasing a new version in the not too distant future that fixes many of the bugs.
I have just checked and I can confirm that if iCal isn't open, then the item doesn't get correctly added to the calendar. It appears the 'Create iCal Event' action currently only works when iCal is open. In order to select the calendar you would like to add your event to, you must alter the calendar item that appears in the 3rd pane of Quicksilver.
