I get this same thing from time to time - there's no logic as to when or why it 
happens.  I have QS (ß71 on OS X 10.8.2) in my login items and sometimes it 
happens on boot, and sometimes it doesn't.

My solution is to launch Terminal and type

killall Dock

That seems to kill the second instance - although even that's an anomaly since 
it indicates there's not actually a second instance of QS running, just a 
second instance of its logo appearing as though it's running in the dock.  (Why 
would re-launching the dock stop an app from running?).  Activity monitor only 
shows one running instance of the app if I launch that and look prior to the 
above Terminal command.

Anyway... this isn't a permanent solution, but quickly rids me of the annoyance 
on those occasions it occurs.
