The hide box is also ticked.
Definitely do not enable the Hide setting in Login Items. This is known to cause problems. It’s probably starting, but in a useless state until you force it to the front somehow. If that doesn’t work, try removing Quicksilver and adding it again (either from System Preferences or by using the Start at login pref).
