I ran into the same issue, once I full-screened an app switching to it with 
quicksilver would sometimes just show the title bar of the app but not 
bring the full-screen window to the front.

But I found a fix, at least in Maverick (I don't have a Mountain Lion 
machine to test with), there's an option under Mission Control Preference 
Pane. "When switching to an application, switch to a Space with open 
windows for the application" Checking this has seemed to fix the issue.
