This often happens when -[NSImage setFlipped:] is used. According to Apple’s Docs:

I’d guess that if you got rid of setFlipped: and replaced it with the suggested method (where needed) it’ll magically sort out the problem. I know we had this problem with QS itself, and I fixed it by just removing (almost) all setFlipped calls.
