I just had the same thing happen to be. Quitting and relaunching QS did not fix it. In my case, neither an *.ods (LibreOffice) document nor *.pdf would offer open. Also, the documents would show the generic blank-paper icon. I went to QS, and chose the catalog that contained the documents, and rescanned.
